I purchased your Full-Spectrum Doula Intake Forms, and wanted to reach out to extend my gratitude for the thought, intention, love, care and wisdom that was poured into the creation of these forms. I had a client complete both the Labor & Birth and General Intake forms and I feel like I was able to gather so much important information on how to provide care. I'm not sure if I would have been able to do that if I hadn't purchased and used these forms.
